The “Supporting Care Givers in the Community” coffee chat provides an informal and engaging opportunity to gather and discuss available community resources and services to aid in your caregiving journey.
This week's conversation will focus on accessing essential services through Ontario Health at Home, which offers support for both the caregiver and the care recipient, as well as exploring other community partner programs.
Caregivers will learn about processes for obtaining home care, specialized programs, in hospital coordination and the Long-Term Care process.
